Definition and Purpose
Outreach is defined by the Massachusetts Executive Office of Senior Affairs as "client finding efforts and activities by the Council on Aging to identify potential participants and introduce existing services and benefits." During an Outreach contact, information is shared and an assessment of a senior's needs is made. The needs of some clients (seniors) may require on-going case management.
Outreach is helping the community's older citizens remain safely in their homes by advocating for the kinds of services that will enhance their dignity, supporting their independence and promoting their general health and well-being. It is also a goal of the Outreach Program to acquaint all the seniors in the community with the kind of information that can broaden their knowledge of seniors' issues and assist them in making informed decisions about the critical areas of their personal lives.
