Request for Closing Statement Form:

Final/Closing Statement for Sale of Property.

Call DPW 508-966-5816 to arrange an appointment for the final meter read. In compliance with the State’s Lead Service Line reporting requirements, the Department of Public Works will need access to the property to identify the water service line material.

During this appointment, the DPW will also perform the following:

-Inspect for non-conforming plumbing and discharge.

-Update the water meter and transmitter, if necessary (no charge)

-Record water meter read for closing statement.

This appointment should be scheduled within 3 to 10 days of the closing date.

After scheduling an appointment, download the Request for Closing Statement form and complete. Take pictures of any trash-recycling bins on the property. The pictures should show the serial numbers. Return the completed form and the bin pictures to DPWOffice@BellinghamMA.org

Closing Statement Form

Pool Abatement Form:

A form for SEWER CUSTOMERS ONLY. The abatement will be taken off your sewer consumption. Failure to furnish this information immediately after pool is filled will nulify any abatement request.

Please take pictures of the pool BEFORE and the water meter BEFORE you fill. Then, upon filling the pool, take a picture AFTER and the water meter AFTER. Make sure all numbers are readable in the pictures. These will need to be sent in with this appliacation. Also, follow all directions on the form and submit to the DPW in person or via email.

ADUs in Bellingham - DPW perspective


For certain types of Accessory Dwelling Units proposed
in Bellingham, you may not need extra utilities or trash
service and above garage and stand along units will
need services. The Requirements document will help
guide you and the Checklist will be for further services
required.
As of 9/1/25, because these units are new in town, we
are evaluating each submittal on a case by case basis.

You should download both documents for your project.
If plans are submitted with your application with
Inspectional Services, we'll use those to determine water,
sewer, and trash services that may be required. A meeting
with DPW may be required.
